The physicians and staff of Surgical Associates Southern Indiana, PSC (SASI) would like to take this opportunity to welcome you as a patient and acquaint you with some policies of our practice.
Knowing that a visit to the doctor’s office is almost certain to cause some degree of anxiety, we wish to make your time with us as pleasant and productive as possible. If you have any questions regarding your treatment, billing policies, etc., please be sure to ask any one of our staff members.
In order to facilitate your time with us to the fullest, we ask that you please arrive 10 minutes early to complete the necessary paperwork. You may download the patient registration forms below, complete and bring with you at the time of your visit.
If you have had any testing done (x-rays, Ultrasound, CT Scans, Mammograms, etc.,) please be sure to bring a copy of those films with you to our office at the time of your visit. You may also ask your primary care physician to fax copies of any reports to our office prior to your visit. Our fax number is (812) 283-6330.
If you are currently taking any medications, please bring a list of them to your appointment. This list should include the name of the medication, the current dosage, and how many times per day you take each medicine.

When you arrive at our office you will be greeted by our receptionist. It is her job to make you feel comfortable and welcome while having you fill out the necessary paperwork, and to confirm your insurance information.
You will be taken to one of our exam rooms by our medical assistant, who will take your medical history. Please feel free to mention any special concerns you may have. You are welcome to have a family member accompany you to the exam room.
Your surgeon will then talk with you and examine you. He/She will go over any recommendations with you and your family. Please ask any questions you may have. The more informed you are the better you will do.
If surgery is planned, you will be asked to sit down with our surgery scheduler to choose a convenient time for your procedure. We will do our best to accommodate your schedule. Please keep in mind that our surgeons have both office time and time in which they are in the operating room. All preparation instructions will be given to you at this time.
At the time your surgery is scheduled, one of our insurance specialists may discuss concerns about your insurance coverage, and the potential need to set up payment arrangements.
